## Why human-in-the-loop matters

AI systems make mistakes, and some mistakes are costly or hard to undo. Placing a person at the right checkpoint catches errors, keeps accountability clear and builds trust in the system over time.

## How human-in-the-loop works

Teams decide which actions the AI can take alone, which need review and which a person must always do. Low-risk, reversible steps can run automatically. High-impact steps, such as payments or customer messages, wait for approval.

Corrections from reviewers can feed back into evaluation sets to improve the system.

## Common human-in-the-loop mistakes

Reviewing everything, which turns people into rubber stamps. Reviewing nothing, which hides errors. Giving reviewers too little context to make a real decision.

It does not have to. If review is limited to high-impact steps and reviewers get the right context, most work still flows automatically.
- Can the human step be removed later?
